Welcome to the list.

1) on a 98, you have a viscous clutch fan. They can be noisy (roar) at
times. It is typical that this will occur on a cold startup. At least you
know its working.
2) this is a common problem. There is a TSB on this. But since its out of
warranty, you'll have to pay to get it fixed if you want. More of an
annoyance as opposed to a problem.
3) I'm not sure about an aftermarket fob. You can get a replacement from
the dealer (shouldn't be too expensive), and they can set it to work for
your truck. If they want an arm and a leg, come back here. I'm sure
someone has figured a way around this.
