RE: VIN/key programming

Combo key, 2006 PT, only 1 existing key on hand. What the going rate for
the key blank, cut and program?
I assume the car needs to be at the dealer for programming? Or can they cut
and program the key without
the car and I do the cycle-key in- out- wait thing myself?
