As far as fuses/fuse panel under the dash are conerned, try this:
-Heater Blower Motor; Check fuse #1(30 AMP Green)
-Heater Controls, Check fuse #13(4 AMP Pink)

As far a relays(under the hood) are concerned, just try this:
-Look at the relay distribution center under the hood on the driver's side
fenderwell, it is a black plastic box, just pop the top off. Look at the
underneath side of the cap, and it should explain what relays are for what.

I do not see a relay for the Heater Fan listed anywhere for my application;
are you sure it is a relay which is bad and not a fuse? Hope this might help
you out somewhat. Let me know what ends up working.
