Well, you'll probably do what i'm going to do next 4x4 trip--put the spare
in the bed before you hit the mud.
But, as far as switching tires from rims, i have only seen it done on large
machinery (huge dumptruck with about a 4 foot tire) and in a program on the
learning channel.
The guy changing the tire on the dumptruck was using a crowbar to get the
lip of the tire or the rim. Not the best plan if you like your shiny rims.
And on that TV show, they were using heavily modified Toyota Landcruisers to
cross the Antarctic--really cool show. But they had this really neat trick
of putting a bit of fuel in the tire when it was out of air on the rim, and
they lit it. The air inside expanded from the heat, and the tire was set on
the rim.

Now, i don't recommend either of these procedures, especially the second.
But there's a couple ways that could be tried if you were facing an extreme
emergency.
