I've noticed a similar thing on my 2000 4.7, but not necessarily at
startup. When it's cold with the trans in DRIVE, but not moving (foot on
brake), it wants to idle around 450-500 and it's rough. It's almost like
the torque converter isn't doing its job because if I shift to neutral it
idles fine at 600. Shift back to Drive and same problem. Once everything
is warmed up the problem (mostly) goes away. It's actually almost stalled
on me a couple times.

The dealer says everything is fine, but I'm skeptical. Let's just say they
haven't earned my respect in the past.
