  I don't know the exact parameters that trigger this, but my understanding
is that basically, the O2 sensor showed the truck being so rich that the
PCM had to go WAY lean to try to bring it back to 14.7:1. The amount it had
to lean out the engine was leaner than some pre-set threshold, so it triggered
a DTC. I don't know if it actually tries to configure the engine this
lean, or if it catches it first and doesn't reduce the fuel by that much,
but basically the computer is telling you "I shouldn't have to cut the fuel
this much; something must be wrong."

  Just conjecture here, but perhaps something is wrong with your engine
coolant temp sensor, causing a really cold reading, which is causing the
PCM to richen up the mixture to compensate, the O2 sensor is showing
its way to rich, and the PCM then tries to compensate by leaning it way
out; fighting with itself, more or less... ? Just a guess.
