This worked on a Chevy, so I'm only assuming Dodge can do it also. You
should be able to go to a dealer, give them your V.I.N., and they can print
it up for you. If you're in good with them, they might not charge you for
it. Otherwise, it shouldn't be more than about 20 bucks or so.
