Its likely partly the gaskets settling and wearing out, and the more you
tighten them, the worse it will get. It may be time to replace the gaskets
and torque the bolts down properly.

I don't know what causes it, but I'm assuming vibration. You could just
get a jug of blue loctite and you'd never have a problem again.
