Weird Fuel Problem

I've had this problem for pushing a year and a half, and at times it
gets better but then I'll be on a trip and it'll bite me in the ass.
It started when I was driving from Oregon to Michigan in Aug 06. I
accidentlly bought some 85 octane fuel in Nevada (at the same stop I
also got food poisoning) and I got all of about 11 mpg from West
Wendover, NV to Salt Lake City, and if you're familiar with the route
there is a reason why they call it the salt flats. I got premium in
SLC and got a good 17.8 mpg driving across the rockies then in
Nebraska about 20 miles shy of my scheduled fuel stop it started
chugging on me and I could only do about 45 with the peddle all the
way down with it coughing and sputtering the whole way. I stopped at a
rest area and just the sloshing from turning and stopping cleared out
the problem until I was back on the freeway, I put more gas in and the
problem cleared up but the gas mileage would usually be significantly
lower for the next few tanks after this. I've tried ever imaginable
fuel additive to clear it up and nothing has helped. Ever since
somewhere between 1/8 and 1/4 of a tank it does this exact same
things, although sometimes it's so bad that all it do is maintain 25
and accelerating at all is virtually impossible.

The truck is a 1994 V-8 4x4 with 149,000 miles on it. I've done some
research and I know the fuel filter is part of the fuel pump and I'm
assuming it's original. Any ideas?
