Just thought I'd offer an update.

Turns out that cheap offshore intermediate shafts have a very, very
short lifespan. I'd replaced this in May 2010 and turns out this was the
issue.

Ordered an OEM unit ($230 instead of $130, but still less than the $450
they want in Canada) and 98% of the slop is gone.

There's a tiny bit of play in one ball joint and one tie-rod. These are
next if I'm keeping the truck.

I'm pretty much left with an excellent and powerful engine backed up by
a noisy 5 SPD transmission, surrounded by a rusting hulk.
