I could be wrong, but this problem sounds to me like the classic noisy
fan clutch problem that has been around since the 92's came out. I've
got a 92 with the 318, 4sp auto, and it is extremely loud when first
started. If you allow the truck to warm up for a couple of minutes, the
noise does not happen. I occasionally get complaints from my neighbors
when I leave at 5:30 in the morning, especially in the summer time when
they have windows open. If I stayed there to warm it up, I'm sure they'd
complain about me revving it up to warm it up. :-)
