I'd check the underside of the throttle plates (especially near the edges)
for carbon build-up. I'd also check the IAC sensor for carbonation.
Do a cleaning on both while you're at it (I use brake cleaning fluid
but use whatever you have).

Surging at idle is generally a sign that the TPS is faulty. You said
that it has been scanned. Check to see that the wiring to the TPS is aok
at the connector. If you want to retest the TPS, make sure the truck
is in Park. As the engine speed surges the TPS volts will increase by
.2 to .3 volts without a change in throttle position. If this is the
case, replace the TPS switch.

Normally O2 sensors last about 60,000 mi. If they are fouled, they
will send signals indicating a constant lean condition (low voltage
of 450mv or less all the time).
