Trouble with Truck

Hey all,

I have been having a problem with my truck for about a month now. Whenever
I reach 2500rpm i get a real loud rattling noise from under my hood...and
also when at idle my engine is really rough....it shakes the whole
truck....anyone have any clue to what this could be?
