I'd have the battery load tested. Also check to make sure your plug wires are
secure in their looms and not resting on the exhaust manifold or something. I
had a wire "pop out" and rest on my headers which caused the engine to run
rough at speed... Everything else sounds like battery problems.

OK, this problem has just developed in the past week or so...
1) turns over hard when cold, like a low battery. catches after a couple
of cranks like normal and runs fine.
2) when coming off of the throttle and taking it out of gear, such as
when approaching an intersection, it returns to an idle of about 500,
instead of 900. it runs a little rough at this speed, but does not
stall. I blip the throttle and it returns to idling at 900 again.

This afternoon, I changed the oil, filter, and spark plugs, and
disconnected and inspected the battery. When I started it back up, it
turned over really hard, and took 30 seconds to catch. Wouldn't hold
the idle, kept stalling out unless I held the throttle. After a minute
or so, it held the idle, at 900, by itself. Took it for a 35 mile test
drive, mostly on the highway, ran fine, except for the 500rpm idle
thing, and 1 small backfire at the beginning, which it has never done
before.

The battery looks fine, condition indicator is green, all lights in
the truck are strong, alternator gauge reads dead-on 14 1/2 since I
bought the truck, no idiot lights are on...any ideas? some oxygen sensor
somewhere? Starter going bad? Have the battery checked anyway? Wait
until the ECM reacclimates itself, since I just unhooked the battery
this afternoon? Help...
