Seen it out here on I-90 in NY. And a few other places. Even if it's
not posted, check your state's laws; there's probably something there
about a general minimum on certain classes of roads (X uunder the posted
limit).

Depends on the conditions. All limits (as far as I know) are
"conditions permitting" - if the weather's crappy and you're doing 35
because that's all that's safe, you shouldn't be ticketed.

Or because it's rarely as much of an issue as a maximum speed can be,
and your state may have one of the aforementioned general guidelines.
