Has anyone replaced these 'frame' brackets before? If so.. how?

Jack, the bolts have, for lack of a better term, a knurled section next to
the bolt head.
They are pressed into the bracket. To install the "factory" tow hook kit,
the bolts must be removed from the bracket either by slugging with a big
hammer and good aim or by pressing them out.
I used a combination of a large "C" clamp, a pipe wrench to turn it, and a
large nut with a hole in it large enough to surround the bolt head.
The entire process takes too much space to describe but it takes three hands
and a lot of time and patience.
I managed to slug four of them loose and pressed out the last two that I
couldn't get near with the hammer.
