Have you considered safety-wiring the bolt [a-la what they do on motorcycles]? That would stop it from backing out. You drill a small hole in the head of the bolt, thread some stainless wire through it, twist the wire tight, and anchor it some-place convenient. I know that's a simplistic explanation, if you need more detail, ask, or serch "motorcycle safety wiring" in the search engine of your choice... HTH.
