If the shaft is bent, even slightly, my choice would be to have it replaced
rather than rebalanced.
Check around, you should be able to find a shop that can fabricate a new
shaft with any kind of u-joints that you want.
My mechanic recommended a local shop and they are busy full time replacing
shafts and joints for heavy trucks. The work was first rate and reasonably
priced.
They had a section at the end of the counter for the really scary stuff.
It's amazing how badly torn up a u-joint for a ten-wheeler can get.
