Gut feeling is ignition coil since it's (as you wrote) worse under a load.
Timing chain can also cause this though (loose/poor timing control).

Starting at 2000 RPM my truck will miss and back fire, I changed
cap/rotor,wires,plugs, reset PCM(no codes). Seems to be worse under load.
Accel and idle feels same.
I asked my local dealer buddies and they are stumped a little about this.
What's your guys input?
