Headers replace the exhaust manifolds... exhaust manifolds basically just dump
the exhaust at a 90 degree angle (ouch...) into one passage which works as a
collector. It isn't very efficient at all, but keeps noise down and they are
very cheap to make in mass quantity.

Headers provide a smooth transition from the engine to the exhaust pipes and
they usually provide larger diameter tubes for the exhaust to flow through....
each cylinder has it's own tube that gradually meet at the collector for a
less turbulant exhaust flow.... this is just like replacing a clogged air
filter or something of that effect - so unless there's a major restriction in
your exhaust (y-pipe?), you should notice a nice boost in power.

Cat-back exhaust systems are just that... a replacement for your exhaust
system from the catalytic converter on back. They're usually of a larger
diameter than the stock exhaust system, and are usually mandrel bent for an
even better exhaust flow.

You should be careful though when selecting replacement exhaust system parts
if you value your low end torque. (For eg; if you tow alot and your truck is
an automatic) Bigger is not always better....
