We got this with our '98. From the information I've gathered, it "just does
that" if you drive them off-road and take some sharp bumps or get the frame
twisted (for example, right front and left rear tires much higher than the
other corners). I never knew it before, but this is apparently the hazard
of four-wheeling in a long wheelbase pickup.
If you haven't been doing any of that, there is a TSB out for cab mounting
bolts. I believe it only affects a small number of '98s, but it might be
worth asking about.
